Yes, because the Elgato picks up the HDMI signal that goes to the TV, this can also be recorded. The Nintendo Switch simply has to be connected to the Elagto Box via the docking station and from there to the TV.

In the settings Audio>Audio device> Select the Tv for output. But then you won't hear anything on your headset... If you really want to continue streaming ONLY on the PS4, then no such device will help you to improve the stream. It also depends on what you mean by "improving the stream". If you are willing to create overlays and use animations/transitions etc. for your streams, then you can do this with a software like OBS Studio or similar and integrate your PS4 gameplay via capture card. THAT would significantly enhance your stream. And for this case, I can highly recommend the HD60S+. TL;DR: without a PC and streaming software, no device will help to "enhance" a stream from the PS4.

You don't need two monitors just to use the product. The HD60S+ is hung between your console and the monitor and also connected to the PC with a USB-C cable. This gives you a stream on the PC of what you are currently doing on the console. If you are working with a monitor, you can simply switch the input between PC and console. The HD60S+ is also a low-latency capture card, so you can do many things directly from the stream on the PC (no switching on the monitor for console input), but there are sometimes slight delays. On the PC, you only need the "4K Capture Utility", a software that Elgato makes available online. It is already equipped with a recording function, so you don't need any additional software.
First of all, for clarification: the HDMI connection of the PS4 is not an input, but an output. The solution is very simple: the HDMI output of the PS4 is connected to the input of the Elgato, and the HDMI output of this device is connected to the input of the audio adapter. I can't answer the question quite conclusively because I don't own a MacBook. I use the Elgato Game Capture HD60S+ to use a normal video camera as a webcam. This means that I feed the video signal from the camera via HDMI into the Elgato and transfer it from there via USB to my desktop Windows PC. This process is completely problem-free, and again it is two independent devices (camera and PC). So if the PC has an HDMI output and the MacBook has a USB port, I think the whole thing should work.
The difference lies in the chipset used and the features available. The HD60S Plus appears under Windows as a UVC device, i.e. any software such as OBS etc. can access the device directly without drivers. The HD60S requires a special driver and software (hence the hassle of support under macOS).
