Can this cable be used to connect a MacBook Pro (mid 2012) to a new iMac 21.5 5k (2016)?

The iMac with 21.5-inch screen is the "4K" model (only the larger 27-inch iMac has 5K resolution).
There is (so far) no 2016 model of these iMacs; the currently sold variant is model year 2015.

The 2015 iMacs have a Thunderbolt 2 port; the MacBook Pro (mid-2012) has a first-generation Thunderbolt port. Both Thunderbolt ports have the same connector. The cable therefore basically fits.

However, the iMac 4K (2015) does not support "Target Display Mode". The Thunderbolt connection can therefore be used for data transfer, but not to use the iMac as an additional monitor for the MacBook Pro.

In my experience, the iMac cannot be used as a monitor.
I tried this with a DP adapter (to HDMI), which failed miserably.

I think what you mean only works with a Thunderbolt display.
