Does the arm come with the base column and laptop shelf? If not, where can I find them?

No, this is just the extension. The base is just called "Monitor arm LX" (look for the colour and table mounting).
I have combined with this one: Ergotron LX monitor arm with patented CF technology (table, 34")

Laptop holder is also separate: Ergotron LX notebook shelf
