I tend to get "mouse arm" and am therefore always on the lookout for ergonomic mice. I use both hands to distribute the load: A high-quality conventional mouse on the right for precision work and various vertical mice on the left, which never completely satisfied me in terms of quality, but reduced the mouse arm problem. As my current vertical mouse needs to be replaced (cheap scroll wheel...), I'm now giving this index trackball mouse a chance because it can be used with both hands.
I found the changeover surprisingly easy, e.g. when browsing I hardly notice the change, but when drawing lines are (still?) shaky. I love the scroll mechanism: you turn the ball like a rotary knob. It's much more ergonomic than a conventional scroll wheel and works really well.
For my wrist, this mouse is not an improvement on conventional mice, my wrist is bent due to the high ball despite the flat mouse design. That's why I'm going to get an armrest.
One small point of criticism is the placement of the mouse buttons and their pressure point. If I place my hand so that my fingertips are on the ball, my thumb and little finger don't reach the ideal pressure point of the left and right click, which is a little further forwards, so I have to use a little more force than is ideal for me to make a click. I can't reach the front two buttons at all without lifting all my fingers off the trackball.

I have had trackballs since they exist. This one is simply the best.
