1: A cable is included but please buy a better one. I only had problems with the included cable. (Thermaltake has a good and well-modelled riser cable available for purchase). 2: Performance loss is zero with the separate cable. With the included cable expect at least 30% loss. And a lot of bluescreens.

No fixture is included. The back has pre-set hole spacing with M6x1 thread, so you can use it with VESA wall mounts (similar to a monitor or TV). I recommend using 200x100.
