I've never had it in extreme use, but it did get quite hot in the case... I got a 5V fan and drilled out the case to mount it, which helps a lot... I think it could work even better with additional heatsinks.

You probably don't really "need" the Raspberry PI. But you can implement many cool projects with it, you can find endless examples on YouTube and Google. Drones, robots, measuring stations (weather / temperature, etc.), e-displays, RetroPI (emulators), SmartHome, web servers, etc. Or you can simply use the PI as a (Linux) computer.

Normally, i would say Yes, take this: Raspberry Pi Official Raspberry Pi 4 Case Fan (Heatsink) But it is may not compatible with the Okdo-Case... But there are some other cases on the market with cooling-fan build-in, just google it ;-) And personaly i use my 3b (the one with the big heat-problem) 7x24 for streaming as a mediaserver and fileserver... no cooling needed, only a crown-job to reboot it all 2-3 days because of the ram that fills up quickly... So i would say for "normal" usage as mine, it should be fine without fan...

As soon as the Pi is used in a case, the heat development is noticeably higher. In my experience, however, this doesn't really make a difference for browsing/video playback. I mounted a larger passive heat sink on mine (you can find it relatively cheaply), but that's only because mine is under constant load.
