Does the transmitter have to be able to send out Bluetooth, or does it work like a mouse with a small (where to connect?) additional device? (We have an old B&O system without digital connections. Only fairly normal analogue sound outputs).

It can only receive Bluetooth. The transmission problem must be solved with an audio Bluetooth adapter, which is available - no problem.
