The description is not entirely clear - do I now need an external power supply (at the doorbell) or is it all completely wireless?

it is wireless and has a battery...spare battery is recommended because you can then simply switch it on as soon as it is empty...the one with cable or without battery can be found here: Arlo Essential Video Doorbell (App, WLAN)
