Four of the six photos show three pre-installed fans but the description only says two, so which is it then?

It has 2. There is a difference between RBG and non-RBG controller version:
PH-EC400ATG_BK01: 2 Fans pre-installed
PH-EC400ATG_DBK/DWT: 3 Fans pre-installed
