Does it come with the nut that you have attached to the gear bridge or do you have to buy it separately?

The nut is not included, but in my opinion it is not necessary because the mount is inserted into the hole and then the screw is tightened on the mount, which then expands the insert and holds it in place.
No additional nuts etc. are required. The mount is inserted into the existing hole in the triple clamp and fixed in place by spreading the shaft.
You have to buy them separately.
