For our kids we have a Watch subscription with Sunrise. They have their own numbers. You will need an iPhone to which the watch is paired, not sure you can avoid that. Setup seems easier if that iPhone is using same provider, but i managed to make it work also with non-matching providers.

The emergency feature on Apple Watch, including Fall Detection and Emergency SOS, does not require a special subscription agreement to work. However, it is required that the Apple Watch or nearby iPhone has a mobile phone connection or a Wi-Fi connection with internet access to access emergency services. You can also store emergency contacts who will be notified when you activate the SOS emergency call or when the fall detection detects a serious fall and automatically makes an emergency call. These contacts will receive a text message with your current location and will receive updates if your location changes.
